Sinisa Mihajlovic was born to a Bosnian Serb father called Bogdan Mihajlovic and a Croat mother called Viktorija who worked in a shoe factory.

Sinisa in his early football career began competing for NK Borovo. He later joined FK Vojvodina before moving to Red Star Belgrade in 1990, where they won the 1990–1991 UEFA Champions League and the Intercontinental Cup 1991.

Mihajlovic moved to the Roma football team in 1992 and spent two seasons there. Mihajlovi signed on with Sampdoria in 1994. Lazio was the team he played for after that.

They won the 1999 UEFA Super Cup, two Coppa Italia, two Supercoppa Italiana, the 1998/1999 UEFA Cup Winners Cup title, and Serie A in the 2005/2006 season. Sinisa then joined Inter Milan and played from 2004 to 2006 before retiring.
