Otamendi began his career in the Primera Divisiónwith Club Atlético Vélez Sarsfield, making his debut on 10 May 2008 in a 2-1 home victory against Rosario Central during the Clausura tournament.

He scored his first professional goal in the 2009 Apertura, helping his team secure a 3-1 win over Arsenal de Sarandí. In August 2010, Otamendi was transferred to FC Porto in Portugal for a fee of €4 million.

He signed a five-year contract with the club, while Vélez retained 50% of his registration rights. Porto had the option to acquire the remaining 50% for an additional €4 million before September 2011, which they exercised. Otamendi played an important role in Porto’s success, contributing to the team’s back-to-back domestic league titles and playing 30 official games during the season.

In February 2014, he was sold to Valencia CF in La Liga for €12 million and signed a five-year contract starting on 1 July. However, he was immediately loaned to Clube AtléticoMineiro in Brazil.

In August of that year, he officially joined Manchester City on a five-year contract worth £32 million. On 15 September, he made his debut for the team. Throughout his final season with Manchester City, Otamendi appeared in a total of 39 matches.

Notably, he played a crucial role in helping his team secure the League Cup for the third consecutive season in March 2020, which turned out to be his last trophy with the club. In September 2020, Otamendi made a move to Benfica, the rival club of his former team Porto. He signed a three-year agreement with Benfica for a fee of €15 million, while Benfica defender Rúben Dias moved to Manchester City for a higher fee as part of the deal.

Otamendi’s league debut for Benfica took place on 4 October in a 3–2 victory against Farense. Since 2009, he has been a part of the Argentina national team, accumulating over 100 appearances.

Otamendi has represented his country in three FIFA World Cups and four Copas América. After finishing as the runner-up in consecutive Copa América tournaments, he finally clinched the title in the 2021 edition.
